
A takeout box papercraft that can be personalized with your names on the top and any loving message on the fortune within the paper cookie. Take your truelove out on a surprise date or just use as a presentation box for your paper fortune cookie.
The box has the words “Take You Out” on the front and personalization for your names on the top. The handle also folds down to place your fortune cookies inside.
Plan a date ahead of time and give this take out box the week before to build anticipation. Or give just before going out with a fortune hint hidden inside the paper cookie to see if your special someone can guess what it is.
Add a second or third paper fortune cookie into the box if you have 2 activities planned for the date. They can be clues of where you are taking her out for a date, then a dinner location, and then the conclusion of the evening.

If it's been a while since you went out together as a couple, make your date memorable with a printable takeout box and fortune cookie.
This craft can be given multiple times just by changing the fortune message inside the cookie.

The template should be printed on heavy cardstock US Letter or A4 Paper sizes. See paper size comparison for more info on A4 size paper. Check printer settings in the instructions below.
Download the personalized template PDF above. Click file to unzip. Right click on PDF to Open With > Adobe Reader.
Click and highlight the wording in the PDF form fields above and below the "Take You Out" text to personalize with your own names.

Repeat to add your own fortune. The fortune provides a hint as to where you are taking your sweetheart for an upcoming date, what to wear, a day/time on the calendar, etc.

In Adobe Acrobat Reader, select File > Print (pages 2 - 3 only with cardstock paper) and make sure Page Scaling is set to NONE. Repeat for page 4 only (fortune cookie) but use regular paper instead.

Use the paper clip to score the fold lines. The fold line guides are the blue dashed lines. Line up the paper clip on the fold line guide, then place the ruler next to it to keep straight. Push down and slide clip along guide lines.
Repeat for each fold line as well as the center rectangle.

Once all fold lines are scored, use the craft knife and slowly cut straight along the gray lines. Since these lines are short, it may be easier to use this method but if it's difficult to cut straight then use a ruler.
Repeat for the handle.

The result of the cutout is as shown.
Don't forget to cut the gray line just above your name in order to close the box later.

Fold along the score lines first from the center rectangle, the triangle sides and the top tabs.

Place a thin piece of double sided tape or glue on the triangle side tip that is attached to the back of the box as shown.

Place the front triangle flap over the back triangle flap and press together to shape box as shown.
Repeat for the opposite side

Shape the handle by folding along the 2 center score lines as shown.

Fold the bottoms of the handle along the angled score line on both sides.

Fold again on score line for both sides as shown.

Place a thin piece of double sided tape or glue on the bottom tip of the handle. (on the white side)

With the handle facing the back of the box, press firmly into the center of the side as shown.
Repeat for the opposite side.

Once the cookie is complete, place it inside of the box, fold down the top and insert tab to close.

Lift handle to finish and give to your loved one.
The handle can be folded down to open the top of the takeout box easily.
