The template should be printed on standard weight US Letter or A4 Paper sizes. See paper size comparison for more info on A4 size paper. Check printer settings in the instructions below.
Download the personalized template PDF above. Click file to unzip. Right click on PDF to Open With > Adobe Reader.
Click and highlight the wording in the PDF form fields to personalize with your own message.

In Adobe Acrobat Reader, select File > Print. Use standard white paper and input page number to print page 2 only. Make sure Page Scaling is set to None.

Use the paper clip to score the fold line in the center of the diamond shape. The fold line guide is the brown dash line.
Line up the paper clip on the dash line, then place the ruler next to it to keep straight. Push down and slide clip along the dash line only.

Once the fold line is scored, use the craft knife and ruler to cut out the fortune message. When cutting, keep the cut path slightly inside of the line so that you do not see a line around the edges.

Use scissors to cut out the "fortune cookie" and slowly turn the paper into the scissors. Cut as slow or fast as you're comfortable without leaving any white around the edges.

Use a pen along each of the triangles that make up the center diamond. Curl the paper around the pen without creasing or folding. Repeat for the other 3 sides.

Shape the paper after rolling with the pen to keep a slight rounded curl on each end again without creasing.

When all 4 curlings with the pen are complete it should look like this.

Hold the tips in your fingers like this and slowly bring the tips together as shown in the next step. (It is important to shape the cookie first without worrying about the fortune falling out the sides.)

One side should be overlapping into the other slightly.

Now that the fortune cookie has been shaped properly, curl the fortune in half without creasing and place inside with text facing up. Then repeat steps 9 & 10 to close again.

Pinch the pointed tips at the top with your fingers like shown and fold to the right.

The result should look like this. Repeat the same for the other side. These small folds make the cookie look less pointy and also keep it together without using tape or glue.

Done.
